Rosemark Starter: Care Plans

Sometimes known as Care Plans, Activities of Daily Living (ADLs), or simply as task logging, this core feature of Rosemark is centered around tracking what actions were completed during a shift. Every agency engages with care plans at a different level depending on the type of care and requirements associated with them. Some agencies might not use this feature at all while others might be required to by Long Term Care Insurance or Medicaid.

Managing the Global Task List

Each database comes with generic tasks but these tasks can be edited based on your specific business needs. Reach out to the Customer Success team to tailor the task list to your needs. 

 

Creating a Care Plan

Care plans are managed on a per service basis. To add a care plan click the service you want to add it to. Here we are going to click on the HHA service. If there are multiple service types, each will have its own care plan.

 

On the service view click the Care Plan List tab. 

When setting up a new care plan you will see a message saying that there is not a care plan for the HHA ST. Click the gold “Add Care Plan” button.

The system will create a care plan starting on that date. However the Care Plan is still empty. To add tasks to the care plan click the Edit button in the top right.  

Backdating a Care Plan

If you need to adjust the stating date for a care plan reach out to the Customer Success team who can assist you with this. They can be reached at (734) 662-3537 ext. 1 or via email at support@rosemarksystem.com.

 

Tasks are organized into categories that can be determined by the agency. To add a task to a care plan check the corresponding box. If applicable you can add any details regarding level or care or frequency as they pertain to the specific client's needs. Once you have added all of the required tasks click the Save button in the top right. 

At any point you can review the care plan. 

 
 

Editing a Care Plan

Over the course of a client's care you may need to update the care plan based on the changing care needs. To edit a care plan go to the Client's Care Plan list. To update it click the active care plan. 

Click the Edit button in the top right corner. 

Then check, uncheck, and adjust care levels and frequency as needed. Click the Save button in the top right corner once you have made the necessary changes. 

 
 

Viewing Historical Care Plan Records

Rosemark keeps a record of care plans in case you ever need to reference it. To view older care plans uncheck the Show Only Current box and click the Update button. This will then display older care plans that you can select to review.
